Job summary

Universal Health Services is seeking an Intake Counselor in Phoenix, AZ for a full-time day shift (6p–6:30a; 12-hour shifts). The role involves telephonic and in-person risk assessment, crisis intervention, and admission coordination to ensure appropriate levels of care.

The position requires a Master’s degree in Counseling, Psychology, Human Services, or MSW, with licensure preferred. You will work with the hospital team to support referrals, families and patients throughout the intake process.

Qualifications

• Master’s degree in Counseling, Psychology, Human Services field, or MSW.
• Arizona license preferred (Board of Behavioral Health Examiners).
• Experience with intake, assessment, admission and crisis intervention in psychiatric settings.

Responsibilities

• Provide telephonic and in-person risk assessment and crisis intervention.
• Coordinate referral and intake process; determine appropriate level of care.
• Facilitate admission and maintain communication with referral sources, families, patients and unit staff.
• Assist physicians with face-to-face clinical assessments and psychosocial information.
