Job summary

Enhabit Home Health & Hospice is hiring for a Referral Coordinator to support patient referrals and work with branch and sales teams to ensure smooth transitions to services. You will apply Medicare guidelines and leverage technology to support operations.

Must have a high school diploma, 2+ years in health care (1 year in home health/pediatrics) and strong customer service skills; valid driver’s license and dependable transportation are often required for field coordination in our Dallas area.

Qualifications

• High school diploma or equivalent required.
• At least 2 years in health care with 1 year in home health or pediatrics preferred.
• Experience in customer service required.
• Knowledge of Medicare guidelines for home health or hospice.
• Basic technology skills including use of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with branch and sales teams to coordinate and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
