Job summary

ProMedica Flower Hospital is seeking an RN Program Coordinator to oversee care from discharge through follow-ups. You will educate patients, review plans of care, and coordinate testing ordered by providers, while tracking health risk assessments with the care team.

Responsibilities include reviewing preoperative clearances, triaging calls, and providing appropriate health referrals. A BSN and RN license are preferred, with ACLS/BLS and EPIC experience highly valued.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.
• EPIC experience.
