Qualifications

• Healthcare experience in referrals/intake in a home health or hospice environment.
• Nursing background is preferred but not required.
• Demonstrates strong communication, negotiation, and public relations skills.
• Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Directs daily patient referral and intake operations and policy implementation.
• Ensures compliance with state, federal, and Joint Commission requirements.
• Maintains relationships with referral sources and oversees patient admissions to home care.
• Oversees insurance verification and authorization processes.
• Gathers and reports referral statistics and trends.
• Maintains knowledge of contractual relationships and community resources.
