Qualifications

• The Intake Coordinator must have healthcare experience, preferably in referrals/intake in a home health environment.
• A nursing background is preferred but not required.
• Demonstrates good communication, negotiation, and public relations skills.
• Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Direct daily patient referral and intake operations and oversee the establishment and implementation of intake policies.
• Direct implementation of improved work methods and procedures to ensure patients are admitted per policy.
• Establish and maintain positive relationships with current and potential referral sources.
• Ensure seamless transition of patients to home care by coordinating education, initiation of plan of care, and coordination with service providers.
• Oversight of insurance verification and authorization processes to maximize third party reimbursement.
