Mission Hospice & Home Care in San Mateo, CA is seeking an Intake-Information Coordinator to manage referral intake, collect medical records, and gather insurance information while complying with policies and applicable laws.

This full-time role offers a Monday–Sunday schedule with Sunday–Thursday; 8:30am–5:00pm, onsite at our San Mateo office. Mission Hospice & Home Care provides a supportive environment and competitive benefits.

Formación

• Associate degree or equivalent from a two-year college or technical school.
• Minimum two years’ experience performing clerical duties, preferably in a healthcare setting.

Responsabilidades

• Coordinate new intake referrals and gather medical records, hospice order, and patient insurance information.
• Perform data entry, intake into EMR for all referrals received.
• Collect data from referral sources, family, and facilities as needed to complete pre-admission information.
• Follow up on information required for referral and admission to Mission Hospice & Home Care.
• Schedule hospice evaluation/informational and admissions visits efficiently.

We are seeking an Intake-Information Coordinator, to work in compliance with organization policies and procedures and applicable laws and regulations.

Schedule

Sunday – Thursday; 8:30am – 5:00pm

Location

Onsite at our San Mateo, CA office

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

Education/Experience

• Associate degree (A. A.) or equivalent from a two-year college or technical school; or equivalent combination of education and experience
• Minimum two years’ experience performing clerical duties preferably in a healthcare setting

Pay Range

$25.00 – $30.00 hourly
