Job summary

Modivcare Personal Care Services is hiring an Intake Coordinator to manage new client referrals and help clients smoothly transition into home care services.

This fully remote position allows you to work from home while supporting access to care. Applicants may reside in any U.S. state but must be available to work Eastern Time business hours; bilingual English/Spanish is required to communicate with diverse clients and payers.

Qualifications

• Bilingual English/Spanish required.
• 1+ year of intake, referral, authorization, or similar healthcare experience.
• Knowledge of Medicaid eligibility and payer sources.
• High school diploma or equivalent.
• Strong communication, organization, and computer skills.

Responsibilities

• Manage new client referrals by phone, email, and payer portals.
• Gather client information and verify eligibility and payer requirements.
• Obtain authorizations and maintain accurate documentation.
• Coordinate with internal teams to move clients from referral to care.
• Communicate with clients, families, and referral partners in English and Spanish.
• Maintain HIPAA and regulatory compliance.

What You’ll Do

• Manage new client referrals by phone, email, and payer portals
• Gather client information and verify eligibility and payer requirements
• Obtain authorizations and maintain accurate documentation
• Coordinate with internal teams to move clients from referral to care
• Communicate with clients, families, and referral partners in English and Spanish
• Maintain HIPAA and regulatory compliance

What You’ll Need
