Job summary

CareOne Home Health in Salida, CA is seeking an Intake Coordinator to support our growing home health team. You will process referrals, coordinate with clinical staff, referral sources, and the marketing team to ensure patient data is entered into our software, visits are scheduled, and all stakeholders are informed of patient status.

The role involves answering phones, handling intake from referrals, and coordinating Start of Care with appropriate staff.

Qualifications

• 1 year related experience; home health experience required.
• Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Experience with Kinnser EMR (WellSky) preferred.
• Experience with Allscripts preferred.
• Bilingual Spanish – speaking, reading and writing preferred.

Responsibilities

• Answers phones and handles intake inquiries.
• Receives intake information and enters data into the system.
• Completes intake forms and routes for admission approval.
• Assists scheduling of appropriately skilled personnel and updates scheduling in the EMR.
• May schedule replacement workers and input scheduling details.
• Communicates with patients and families.
• Takes referral information when nursing staff unavailable and routes referrals promptly.
• Documents and reports changes in patient status and verifies Start of Care physician.
• Scans and uploads referral documents.
