Job summary

ProSource in Houston, TX is seeking an experienced Intake Coordinator to be the first contact for patients entering our home health care services. You will verify insurance, initiate the intake process, and collaborate with physicians, hospitals, and case managers to ensure timely admissions.

You should have 1–2 years in intake for home health, be proficient with AIDEN/AIDA and Kinnser/WellSky, and possess excellent communication and organizational skills.

Qualifications

• 1–2 years of intake experience in home health care.
• Strong working knowledge of AIDEN/AIDA and Kinnser/WellSky.
• Excellent communication and organizational skills.
• Ability to manage multiple tasks in a fast-paced environment.
• Bilingual abilities are a plus.

Responsibilities

• Manage incoming referrals and initiate the intake process
• Verify insurance and obtain authorizations
• Communicate with physicians, hospitals, and case managers
• Enter patient and physician information into Kinnser (WellSky)
• Work closely with the Patient Care Coordinator to ensure staffing needs are met
• Coordinate with clinical staff to ensure timely admissions
• Ensure all documentation meets Medicare and agency standards
• On-Call on a rotating shift
