Job summary

Community Eldercare Services is seeking an Intake Coordinator to serve as the first point of contact for new patient referrals, coordinating with facilities, clinical teams, financial partners, and admissions professionals to ensure accurate, timely processing focused on patient needs.

In this role you will review referral documentation, screen admission eligibility, and distribute referrals to the most appropriate facilities while maintaining confidentiality and collaboration with care teams

Qualifications

• Associate's degree in business or health related field preferred.
• 2 years' experience in long term/ skilled care admissions preferred.

Responsibilities

• Serve as the primary point of contact for new patient referrals.
• Review referral documentation and screen patients to determine admission eligibility.
• Coordinate the distribution of referrals to facilities that best meet each patient's needs.
• Partner with Clinical and Financial teams and facility admissions teams.
• Build strong, collaborative relationships with facility and admissions teams.
• Ensure referrals are processed promptly, accurately, and confidentially.
