Job summary

Wesley Community Center seeks a FOC Intake and Case Manager to oversee client intake, eligibility assessment, and enrollment in public benefit programs. You will connect clients to resources, provide direct case management, and advocate for access to services, including food, housing, and utilities support.

The role requires coordination with the Financial Opportunity Center and may involve outreach to community partners.

Qualifications

• Minimum 3 years’ work experience in a similar organization coordinating program services and providing direct case management.
• Experience coordinating program services and activities in a nonprofit setting.
• Bilingual English and Spanish preferred; strong written and verbal communication skills.

Responsibilities

• Conduct client intake and assess eligibility for services.
• Assist clients with enrollment in public benefit programs.
• Provide one-on-one case management and advocacy for clients.
• Maintain confidential client files and document monthly activity summaries.
• Refer clients to Wesley programs and wrap-around services as needed.
• Support grant reporting and data management requirements.
