Enhabit Home Health & Hospice in Massachusetts is seeking a referral coordinator to join our expanding team. You will coordinate patient referrals, support branch and sales teams, and ensure compliant transitions to services.

Qualified candidates have a high school diploma, at least two years in healthcare (one year in home health or pediatrics), strong customer service, and familiarity with Medicare guidelines. Valid driver's license and reliable transportation are required.

Qualifications

• High school diploma or equivalent required.
• At least 2 years in healthcare and 1 year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines governing home health/hospice.
• Basic tech skills including mobile device operation.

Responsabilités

• Coordinate patient referrals and act as a resource for referral sources.
• Collaborate with branch and sales teams to ensure coordination and transitions to services.
