Qualifications

• High School diploma or equivalency.
• Bachelor’s degree in related field or equivalent experience preferred.
• Two years’ experience in a health care setting providing intake management or similar duties.
• Bilingual English/Spanish a plus.
• Possess a valid California Driver’s License.

Responsibilities

• Facilitates patient admissions and assists with accessing income benefits and health coverage.
• Collects intake data, prepares admission packets, and coordinates health assessments.
• Maintains confidential patient information and DHS electronic records.
• Coordinates with DHS staff and service providers to link patients to benefits.
