Granite VNA, Inc. in Concord, NH seeks an Intake Assistant to support patient intake for Home Care and Hospice. You will respond to inquiries, enter referrals into the EMR, and coordinate with Concord Hospital referrals.

This in-office role follows a Mon-Wed schedule from 8:00 AM to 4:30 PM and pays $21.00–$25.00 per hour. The position requires attention to detail, data-entry proficiency, and strong communication skills to ensure accurate documentation, privacy compliance, and smooth referral

Qualificações

• Fax Prehab referrals to appropriate agencies.
• Enter verbal orders accurately and timely.
• Enter pending referrals into Referral Manager.
• Maintain knowledge of contracts and insurance for initial authorization.
• Manage Granite VNA referral email group.
• Daily reconciliation of Concord Hospital dischargers and transfers.

JOB SPECIFICATIONS:

Title: Intake Assistant

Location: Concord, NH (In Office)

Schedule: Monday-Wednesday | 8AM - 4:30PM

Pay: $21.00 - $25.00/hour

Position Overview

Reporting to the Chief Strategy Officer, the Intake Assistant is responsible for responding to all inquiries and assisting with the patient intake process for Home Care and Hospice. Coordinates the referral process for new patients.

Essential Functions

• Accurately enters all information for new referrals from Concord Hospital into referral Manager and Electronic Medical Record (EMR)
• Accurately enters all information for resumptions of care.
• Attaches referrals accurately to the Patient Medical Record.
• Faxes Prehab referrals to appropriate agencies.
• Enters verbal orders accurately and timely
• Enters pending referrals accurately and timely into Referral Manager
• Maintains a working knowledge of Granite VNA’s contractual relationships, insurance plans and managed care managers for initial authorization of services
• Manages and oversees the Granite VNA referral email group
• Daily reconciliation of Concord Hospital dischargers and transfers; includes adding the appropriate liaison and event code

Other Job Functions

• Seeks out assistance of the Intake Nurse for support and clarification of unfamiliar areas
• Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.
• Enters weekly time sheet according to agency policy
• Adheres to work schedule and attendance according to agency standard
• Adheres to Telecommunication and Personal Cell Phone Use guideline
• Understands and demonstrates compliance with agency policies and procedures
• Complies with infection control and safety measures in accordance with agency policy and procedure
• Completes all agency’s policies regarding client’s privacy, confidentiality and HIPAA requirements. Adheres to the Agency’s standard of professional appearance and the Agency’s dress code policy
