Job summary

Windsor Laurelwood Center for Behavioral Medicine in Willoughby, OH, seeks an Intake Coordinator to support assessment and referral activities. You will collaborate with medical staff, treatment teams and case managers to coordinate care for adults, adolescents and children receiving inpatient or outpatient services.

The role requires a Master’s degree in social work/psychology/mental health (or RN license) and at least two years of clinical psychiatric experience.

Qualifications

• Master's degree in social work/psychology/mental health or related field (or RN license).
• Minimum of two years direct clinical experience in psychiatric/mental health settings.
• Experience with clinical interviewing, patient assessment, family involvement and treatment planning.

Responsibilities

• Handle intake assessments and referrals for patients at the facility.
• Coordinate with medical staff, treatment teams and external case managers.
• Support treatment planning, discharge planning and care coordination.
