Palo Verde Behavioral Health Hospital in Tucson, AZ seeks an experienced Intake Assessment Counselor/Coordinator to serve as the first point of contact for patients and referral sources, performing clinical assessments and determining ongoing needs.

You will conduct telephonic risk assessments, complete in-person evaluations, and decide level of care in consultation with the hospital psychiatrist, facilitating admissions when required and supporting IOP admissions.

Competenze

• Master's degree in a related field is required.
• Experience: 3-5 years minimum in relevant clinical settings.
• LMSW, LPC, LISAC, or RN licensure preferred.

Mansioni

• The Intake Assessment Counselor/Coordinator is the initial contact for patients and referral sources, providing clinical assessment and determination of ongoing clinical needs.
• Provides telephonic risk assessment and in-person clinical assessment.
• Determines appropriate level of care by consulting with the hospital psychiatrist to determine disposition.
• If Patient is admitted to the hospital, the Counselor facilitates the admission process and provides appropriate clinical information for ongoing treatment.
• Conducts clinical evaluations and admissions for the Intensive Outpatient Program (IOP).
