Job summary

Valle del Sol, Inc. in Phoenix, AZ is seeking a Clinical Assessor to initiate care by completing ADHS mandated assessments, integrated service plans, crisis/risk tools, and intake interviews with clients and guardians when needed.

You will present the Child and Family Team paperwork, support annual comprehensive assessments, treatment plan updates, and staff training, while delivering culturally sensitive behavioral health services in English and Spanish and using EHR systems.

Qualifications

• 18 Years of age or older.
• Bachelor’s or master’s degree in behavioral health (B.H.) related field; or Clinical Intern in bachelor’s or master’s degree behavioral health program in good standing.
• Bachelor’s or master’s degree in non-BH related field AND 1 year of BH work experience; or
• Associate’s degree AND 2 years of BH work experience; or
• High School diploma and 4 years of BH setting experience (1 year direct patient contact).
• Must be bilingual in English and Spanish.
• Must have knowledge to provide behavioral health services and to meet consumer populations’ needs.

Responsibilities

• Meet with patients and guardians, establish rapport, and gather confidential information.
• Determine preliminary diagnosis for intake to form Integrated BH service plan.
• Complete Mental Status Exam, Crisis Plan, Risk Assessment, Strength and Culture Discovery, and overall impressions.
• Explain Child and Family Team process to youth and families; present CFT paperwork.
• Complete Annual Comprehensive Assessments and treatment plan updates; coordinate with care coordinators.
• Assist with staff training as needed.
