Job summary

CovenantHealth at Home with Compassus, Hospice of Lubbock, seeks an Intake Coordinator to manage referrals for home health and hospice programs. You will verify insurance, coordinate with clinical teams, and maintain accurate EMR documentation while fostering strong relationships with patients, families and referral sources.

Responsibilities include prioritizing referrals, entering data into EMR, verifying physician orders and insurance coverage, and supporting staff onboarding.

Qualifications

• 1 year administrative experience in home health or hospice is required.
• Preferred: 1 year intake experience in home health or hospice.
• Preferred: 2 years of clerical tasks in a healthcare setting.

Responsibilities

• Prioritize daily referrals for timely admissions.
• Enter referrals from phone, fax or secure messaging into EMR accurately.
• Answer calls promptly and courteously.
• Transcribe medical and insurance information from referrals.
• Verify physician orders, licenses, and UPIN numbers.
• Evaluate insurance for reimbursement and communicate coverage criteria to patients.
• Collaborate with clinical team to coordinate services.
• Assist orientation for new personnel on referral processes.
• Perform other tasks as assigned.
