Job summary

One Hope United in Illinois seeks an Intact Case Manager to provide comprehensive case management to families in the child welfare system, including assessments, service planning, coordination, and progress monitoring.

You will conduct initial assessments, develop individualized service plans with families and providers, maintain records, and participate in case conferences. A Bachelor's degree in social work or related field and a valid driver's license are required.

We are seeking a dedicated and experienced Intact Case Manager to join our team. The Intact Case Manager will be responsible for providing comprehensive case management services to families involved in the child welfare system. This includes conducting assessments, developing service plans, coordinating services, and monitoring progress towards goals.
