Job summary

One Hope United seeks a dedicated Child Welfare Case Manager in Sebring, FL. The role focuses on direct case management for children, birth and foster families, and primary caregivers across home, school, and community settings.

You will develop and monitor case plans, coordinate with therapists and agencies, and maintain detailed documentation. Candidates should have a Bachelor’s degree (Master’s preferred) in relevant fields and be willing to obtain CWCM certification within a year.

Qualifications

  • Bachelor’s degree required or a Master’s degree in Social Work, Criminology, Psychology, Sociology, Counseling, Special Education, Education, Human Development, Child Development, Family Development, Marriage & Family Therapy, or Nursing from an accredited college or university preferred
  • Must be willing to obtain CWCM certification within 1 year of employment (required)

Responsibilities

  • Provide direct case management services to children, birth families, foster families and primary caregivers in home settings, day care, preschool, and/or school
  • Develop, implement and monitor case plans; coordinate services and resources with mental health professionals and community agencies
  • Maintain documentation, reports, and releases of information; ensure confidentiality and compliance with policy
  • Attend trainings, staffings, court hearings, and team meetings; support families to achieve safety, well-being, and permanency for children

Education

Bachelor’s degree
Master’s degree in Social Work (MSW)
CWCM certification (preferred)

One Hope United works with children of all ages who are wards of the state due to abuse, neglect or abandonment. These children need a stable, dependable, forever home and a family they can call their own. Safe, well-educated and well-cared-for children become adults who contribute to a more prosperous community. Hundreds of abused, neglected or abandoned children need a stable, nurturing family where they can thrive. One Hope United recruits and trains foster parents and provides myriad supportive services to children and families. Foster parents can light a child’s path toward becoming a successful adult.
Case Managers are responsible for providing direct case management services to children, birth families, foster families and primary caregiver(s) in the home setting, day care, preschool, and/or school to ensure they receive services appropriate to their needs, and continuity of care focused on resolving or preventing the issues of child abuse or neglect. The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family and primary caregiver. The Case Manager works as a team member with a therapist, foster parents, primary caregiver, guardian ad litem and other appropriate professionals. The Case Manager will develop, strengthen and maintain a direct working relationship with mental health professionals, community agencies and the child’s own support system.

Essential Duties and Responsibilities
  • Abides by Incident Reporting policy and procedures as applicable
  • Arranges visitation schedule structured upon the completed visitation plan with input from the appropriate parties, supervise visitations and document observations as appropriate
  • Assists in the provision of appropriate information when applicable and accompanies children to appointments, as necessary, to report progress or lack thereof
  • Assists the primary caregiver(s) and/or foster family in implementing tasks and activities according to the case plan
  • Attends and participates in professional development training for professional growth as developed by CBC/Pre Service Training, as required by the state of Florida and in accordance with agency policy and procedures
  • Attends and participates in staffing reviews, administrative reviews/staffing, judicial reviews, permanency staffing, school staffing, CPT staffing, meetings, etc.
  • Completes all non-clinical intake paperwork for case management services according to OHU and CBC policy and procedure.
  • Completes and attends mandatory trainings
  • Develops and maintains appropriate documentation and concurrent planning in accordance with OHU and/or CBC policy and procedures
  • Identifies strengths, limitations, risk factors, and needs, to refer and coordinate the provision of concrete services to the child, primary caregiver, and biological families of children
  • Intervenes and assists OHU/CBC staff in crisis situations
  • Maintains client confidentiality according to established policies, federal and state statutes, as evidenced by obtaining needed releases of information and maintaining updated releases of information
  • Maintains established caseload productivity requirements and standards for direct client contact
  • Maintains face-to-face and telephone contact with each of the assigned eligible children/families in the foster home, primary residence of the child and/or family, school, day care, preschool or any appropriate setting
  • Notifies the biological parents of any changes in status, placement, medical & clinical treatment, etc., relative to the child
  • Provides a written assessment of each family and develop, with the team, a case plan in a timely manner of receiving a new case, provide case plan updates, and family support plans. Monitors and reviews case plans weekly with team members and properly files changes in the case plan in advance with the court, DCF and Lead Agency
  • Provides complete documentation for services rendered within established time frames, including daily log, progress notes, assessments and case plans, and other required documentation
  • Provides on-going support to the primary caregiver(s) and/or foster family to assist the family in providing a safe and secure home environment for the child
  • Provides recommendations to reunite the family, terminate parental rights, remove child from family, and close cases when appropriate according to OHU and/or CBC policy and procedures
  • Refers the child, as appropriate, for medical care, including developmental, psychiatric, medical evaluations, treatment, etc.
  • Tracks dates and attends court with prepared documentation; assists with notification of all appropriate parties to attend court and all appropriate staffing
  • Performs other related duties as assigned
Education, Experience and Skills
  • Bachelor’s degree requiredor a Master’s degree in Social Work, Criminology, Psychology, Sociology, Counseling, Special Education, Education, Human Development, Child Development, Family Development, Marriage & Family Therapy, or Nursing from an accredited college or university preferred
  • State of Florida ChildWelfare Certification (CWCM) preferred
  • must be willing to obtain CWCM certification within 1 year of employmentrequired
  • Must be willing to go through 10 week trainingrequired
  • Must have reliable transportation and willing to drive daily for work related purposes.
