Job summary

One Hope United is seeking a Case Manager in Florida to provide direct case management for children in various settings, coordinating services with families, foster parents, therapists and providers.

You will participate in court proceedings, develop care plans, and monitor safety and well-being, including home visits and collaboration with community agencies to support permanency goals.

Qualifications

  • Bachelor’s degree approved by the Florida Board (e.g. Behavioral Sciences, Community Development, Criminal Justice, Criminology, Education Health Science, Human Development, Human Services, Law, Political Science, Public Administration, Psychology, Social Policy, Sociology)
  • State of Florida ChildWelfare Certification (CWCM) preferred, but not required
  • Willing to obtain CWCM certification within 1 year of employment
  • Willingness to complete 10-week training
  • Reliable transportation and ability to drive daily for work-related purposes
  • Availability to include evenings, weekends and on-call
  • Ability to be culturally diverse

Responsibilities

  • Arranges visitation schedule based on the visitation plan with input from relevant parties, supervises visitations and documents observations
  • Accompanies children to appointments and reports progress or lack thereof
  • Assists caregivers and foster families in carrying out tasks per the case plan
  • Participates in professional development training for growth
  • Identifies strengths, risks, and needs to coordinate services
  • Serves as a conduit of information between parties and providers
  • Conducts home visits to assess child safety, permanency and well-being
  • Develops and implements plans to meet child and family needs
  • Facilitates placement and delivery of services with families
  • Attends court hearings and files required documents
  • Intervenes in crises and supports staff
  • Meets families in homes during evenings, nights, or weekends
  • Performs other duties per job description

Job description

One Hope United works with children of all ages who are wards of the state due to abuse, neglect or abandonment. These children need a stable, dependable, forever home and a family they can call their own. Hundreds of abused, neglected or abandoned children need a stable, nurturing family where they can thrive. One Hope United recruits and trains foster parents and provides myriad supportive services to children and families. Foster parents can light a child’s path toward becoming a successful adult.

Case Managers are responsible for providing direct case management services to children, birth families, foster families and primary caregiver(s) in the home setting, day care, preschool, and/or school to ensure they receive services appropriate to their needs, and continuity of care focused on resolving or preventing the issues of child abuse or neglect. The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family and primary caregiver. The Case Manager works as a team member with a therapist, foster parents, primary caregiver, guardian ad litem and other appropriate professionals. The Case Manager will develop, strengthen and maintain a direct working relationship with mental health professionals, community agencies and the child’s own support system.

Position Responsibilities Include:
  • Arranges visitation schedule structured upon the completed visitation plan with input from the appropriate parties, supervise visitations and document observations as appropriate
  • Assists in the provision of appropriate information when applicable and accompanies children to appointments, as necessary, to report progress or lack thereof
  • Assists the primary caregiver(s) and/or foster family in implementing tasks and activities according to the case plan
  • Attends and participates in professional development training for professional growth as developed by CBC/Pre Service Training, as required by the state of Florida and in accordance with agency policy and procedures
  • Identifies strengths, limitations, risk factors, and needs, to refer and coordinate the provision of concrete services to the child, primary caregiver, and biological families of children
  • Serve as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
  • Conduct visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency and well-being.
  • Develop and implement a plan that meets the needs of the children and the family.
  • Facilitate placement and delivery of services in collaboration with biological, foster and adoptive families.
  • Perform case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.
  • Attend court hearings, prepare and file all court documents within required time frames.
  • Intervenes and assists OHU/CBC staff in crisis situations
  • Meet with families in their homes at various hours of the day including evenings, nights and weekends, in potentially high risk neighborhoods.

Education, Experience and Skills
