Job summary

UC San Diego Health's Inpatient Abdominal Transplant Coordinator RN role supports care across the transplant continuum for liver and kidney patients. You will coordinate referrals, waitlists, evaluations, and post-transplant care, ensuring seamless handoffs across campuses.

You will collaborate with surgeons, hepatologists, nephrologists, and the transplant team to educate patients and families, anticipate needs, and uphold regulatory standards.

Qualifications

• Graduate from an accredited nursing program with active California RN license.
• Minimum two years in acute care RN with transplant experience or transplant coordinator.
• Ability to coordinate care from referral to discharge and communicate with the transplant team.

Responsibilities

• Coordinate kidney and liver transplant inpatients across campuses from referral to post‑transplant follow‑up.
• Escalate patient changes to surgeons, hepatologists, nephrologists and team as needed.
• Educate patients and families on immunosuppressive regimens and post‑transplant care.
• Ensure regulatory and quality compliance with UNOS/CMS/JCAHO standards.
