Job summary

UC San Diego Health is seeking an Inpatient Abdominal Transplant Coordinator RN to ensure continuity of care for liver and kidney transplant patients. You will collaborate with surgeons, hepatologists, nephrologists, and the transplant team to deliver coordinated inpatient care.

The role requires CA RN licensure with at least two years of acute transplant experience, strong communication, and the ability to navigate Epic and Phoenix for charting and post-transplant follow-up.

Qualifications

• RN licensed in California required.
• Minimum two years acute care RN experience with transplant patients or transplant coordinator experience.
• Knowledge of UNOS/CMS/JCAHO standards is preferred.

Responsibilities

• Coordinate kidney and liver transplant inpatients across campuses.
• Provide clinical oversight and escalate patient changes to transplant surgeons.
• Communicate with multidisciplinary team and families; participate in rounds.
• Navigate Epic and Phoenix to access and document patient charts.
• Ensure timely testing, imaging, and discharge planning.
