Qualifications

• Graduate from an accredited school of Nursing.

• Two years of acute care transplant experience or transplant coordination experience.
• Ability to coordinate care from referral to discharge and work with a multidisciplinary team.

Responsibilities

• Coordinate inpatient transplant care across campuses for liver/kidney patients.
• Collaborate with surgeons, hepatologists, nephrologists and care teams.
• Provide patient/family education on transplant processes and regimens.
• Navigate Epic/Phoenix to access and update patient charts and records.
