Job summary

Hackensack Meridian Health is seeking a Transitional Care Advanced Practice Nurse (APN) or Physician Assistant (PA) to provide patient-centered, coordinated care in Brick, NJ. You will work with the hospital team to assess, plan, and implement care for patients with health and safety needs, including inpatient responsibilities for the Center for Care Transformation.

This role requires completion of an approved PA program or NP program, NJ licensure, and board certification; strong communication

Qualifications

• Successful completion of an approved physician's assistant program, accredited by the AMA Council on Medical Education.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Board certification by AANP or ANCC is required.

Responsibilities

• Inpatient responsibilities for the Center for Care Transformation.
• Assist, monitor, and manage day-to-day patient needs under physician supervision.
• Collaborate with the practice/hospital team to assess, plan, and implement care.
