Hackensack Meridian Health in New Jersey seeks an Advanced Practice Nurse or Physician Assistant to deliver ambulatory primary care in a patient-centered model. The role emphasizes collaboration with the practice/hospital team to assess, plan, and implement care for patients with health and safety needs.

The ideal candidate will hold an NP or PA credential, an NJ RN license, and current BLS/ACLS certifications, with prescriptive authority and a collaborative agreement with physicians on staff.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• NPI (National Provider Identifier).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Successful completion of an AMA-accredited physician's assistant program.
