Job summary

Advocate Health Care is seeking a Speech-Language Pathologist to interpret physician referrals and evaluate patients with diverse speech, language, voice, cognitive, and swallowing disorders. You will develop and implement care plans, monitor progress, and adjust treatment to achieve functional goals.

You will manage documentation, discharge planning, and collaboration with the care team, while maintaining productivity standards and ensuring patient safety through adherence to policies.

Qualifications

• Master’s degree in Speech Pathology required.
• Speech Language Pathologist license in state where practicing.
• Basic Life Support (BLS) for Healthcare Providers within 90 days.
• CCC-SLP certification required.

Responsibilities

• Interpret physician referrals and conduct speech, language, voice, cognitive, and swallowing evaluations.
• Establish written plan of care and implement treatment based on evaluation results and goals.
• Monitor outcomes and adjust treatment plans, providing status updates to physicians.
• Plan for discharge, including home programs, teaching, and referrals.
• Complete documentation per policy, accreditation standards, and CMS-like guidelines.
• Manage caseload, coordinate coverage, and participate in performance improvement activities.
• Assist with supervising non-licensed staff and student clinical education.
