Responsibilities

• Conducts and documents detailed psychosocial assessments/evaluations to interpret social, environmental, and medical needs of the patient/family.
• Reassesses, monitors, and documents needs and interventions; ensures medical coverage by securing documentation.
• Provides or supervises plan of care interventions including resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling.
