Job summary

Ochsner Health is seeking a qualified medical social worker to perform bio-psychosocial assessments, coordinate discharge plans, and connect patients with community resources. The role requires strong communication and record-keeping within a multidisciplinary team.

Required MSW (CSWE-accredited) with CSW or RSW certification; LMSW preferred and may need LMSW within 6 months. Two years of hospital experience is preferred, with proficiency in Windows applications and data entry.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW) certification required; LMSW preferred.
• LMSW license preferred or must schedule and pass LMSW exam within 6 months.
• No specific years of experience required; two years in a medical setting preferred.
• Proficient with Windows applications and data entry.

Responsibilities

• Conduct bio-psychosocial assessments and discharge planning.
• Coordinate referrals to community resources and supports.
• Maintain patient records and communicate with the medical team.
• Engage in continuing education to maintain knowledge and certifications.
• Participate in on-call rotations as needed.
• Adapt communication to diverse patient populations.
