Job summary

University of Maryland Medical System is seeking a qualified Social Worker in a case management role to support patients and families with comprehensive psychosocial care.

The position involves psychosocial assessments, crisis intervention, care planning, and coordination of referrals within a multidisciplinary team. A Master's in Social Work (CSWE) and LMSW licensure in Maryland are required, with at least one year of related experience.

Qualifications

• Master's degree in Social Work (CSWE accredited)
• LMSW licensure from Maryland Board of Social Work Examiners
• Minimum one year of related experience is required.

Responsibilities

• Interviews patients to assess functional level and completes biopsychosocial assessments.
• Develops a plan of care and discharge plans reviewed with the interdisciplinary team.
• Provides crisis intervention services and uses de-escalation strategies with patients.
• Assesses social determinants of health and coordinates referrals to improve care.
• Documents all work timely and maintains patient confidentiality per regulations.
