Job summary

Trinity Health seeks a Full Time Inpatient RN Case Manager for weekday day shifts with minimal weekend and holiday requirements. This CARE COORDINATION role coordinates with physicians, staff, and payers to guide patients along the continuum of care toward quality outcomes.

Responsibilities include assessing discharge needs, creating plans with a multidisciplinary team, educating patients and families, and leading rounds.

Qualifications

• Bachelor's degree required; master's preferred.
• 3–5 years hospital case management experience.
• Experience with discharge planning and care coordination.
• ACMA, CMGT-BC, CCM certifications preferred.

Responsibilities

• Assess complex discharge needs and establish a plan with the care team.
• Oversee care coordination for defined patient populations.
• Negotiate resources and educate patient/family on options.
• Demonstrate clinical competence for the patient population.
• Lead daily multidisciplinary rounds and address barriers.
• Facilitate patient education for safe, effective discharge.
