Responsibilities

• Coordinate inpatient care with multidisciplinary team and ensure appropriate discharge planning.
• Assess discharge needs and establish care plans with a multidisciplinary team.
• Lead daily multidisciplinary rounds and facilitate education for safe discharge.
• Support nursing staff and act as patient advocate across departments and with families.
