Job summary

This full-time role is day shift, 40 hours per week, with some weekends (approx 6-8 per year) and occasional holidays, emphasizing patient advocacy, care coordination, and evidence-based practice in a Magnet-designated hospital.

Qualifications

• RN licensure in Michigan is required.
• BSN degree required; ADN with BSN plan within 4 years.
• CM certification preferred.
• Minimum 2 years Case Management in acute care.
• Preferred: 5+ years Case Management experience.

Responsibilities

• Provide case management to patients and families in acute care.
• Oversee discharge planning and utilization management for assigned caseload.
• Coordinate resource use to support cost-effective care.
• Facilitate patient progression through the care continuum.
• Collaborate with physicians and care team on planning.
• Assess patients and develop coordinated care plans.
• Advocate for patients/families and participate in meetings.
• Educate patients and families about plans and resources.
