Job summary

Qualifications

• Must have Bachelor's degree (required) and Master's degree preferred.
• 3-5 years of hospital case management experience required; 6-10 years preferred.
• Active professional certifications (ACMA, CMGT-BC, CCM) preferred.

Responsibilities

• Coordinate care with multidisciplinary team to ensure efficient, cost-effective discharge planning.
• Assess complex discharge needs and establish plans of care with the team.
• Lead daily rounds, address barriers, and educate patients/families for safe discharge.
• Support staff development, mentorship, and participation in professional programs.
