Qualifications

• Current and valid NYS RN license.
• Bachelor of Science in Nursing or willingness to pursue a defined plan for matriculation in program.
• 2 years of experience in acute setting.

Responsibilities

• Collaboratively manage the progression of patients' care with the health care team using evidence-based guidelines.
• Engage patient and care team in joint management of outcomes such as disease progression, LOS, and post-acute transition.
• Ensure optimal communication and utilization of clinical resources to manage patient flow across the continuum.
• Serve as a resource and educator to staff, patients, and families.
