Description:The position does not involve clinical responsibilities.Candidates must have a minimum of 2 full years of experience to be considered.General certification requirements include BLS.Certification as a Certified Case Manager (CCM) is preferred but not required.The case load consists of 8-10 patients per day.The EMR system used is Meditech.This position involves Utilization Review (UR) and discharge planning.UR responsibilities include Notices of Admission (NOA).The coverage areas include inpatient, observation, and swing bed.Common patient types include those with COPD, CHF, and general debility for swing bed patients.No Case Management Manager or Supervisor is present on each shift.Shifts and scheduling are Monday through Friday from 0700-1530.There are no weekend, holiday, or call requirements.Pharmacy involvement is included for discharge planning.The orientation includes 2-3 shifts of department-specific training.There will be a mentor available to answer any questions that arise during the assignment.The cancellation policy allows for up to 4 shifts to be canceled during the assignment.The charting system in use is Meditech.
