Job summary

Southern NH Medical Centers is seeking an Acute Care Case Manager to guide patients and families through discharge planning, ensuring a safe and smooth transition from hospital to next level of care.

The role requires a healthcare-related bachelor’s degree or equivalent experience, 3–5 years in case management, EMR documentation experience preferred, and ASN/BSN is welcome but not required; seven years of experience counts in lieu of a degree.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• ASN/BSN preferred but not required.
• 3–5 years of case management-related health care experience.
• Experience with patient documentation in an EMR system preferred.

Responsibilities

• Assesses and identifies patients' needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Utilization Reviews.
