Job summary

Qualifications

Responsibilities

• Collaborates with the health care team to coordinate the plan of care.
• Communicates with patient, family, and care team to address goals.
• Develops a care plan with goals, timelines, and required services across the continuum.
• Monitors outcomes, documents progress, and recommends plan modifications as needed.
• Manages patient caseload and participates in performance improvement activities.
