Job summary

Augusta Health in Fishersville, VA is seeking an Inpatient RN Case Manager to coordinate patient care and discharge plans aligned with patients' goals of care. You will assess needs and guide safe transitions across care settings with strong interprofessional collaboration.

A nursing degree with Virginia RN license and 2+ years in acute care is required; BS/MS in Nursing and case-management certification are preferred.

Qualifications

• Bachelor's degree in nursing or equivalent experience.
• Registered nurse license in Virginia or a Compact state.
• Minimum of 2 years in an acute hospital setting.
• BS or MS in Nursing (preferred).
• National certification in case management (preferred).

Responsibilities

• Coordinate patient care and discharge planning aligned with patients' goals of care.
• Assess needs and guide safe transitions across care settings.
• Collaborate with interdisciplinary teams to ensure timely, safe transitions.
• Identify and report quality-of-care and risk management issues.
