Qualifications

• Bachelor's degree in Nursing is required.
• Current and valid RN license in Virginia or compact state required.
• Ambulatory Care Nursing Certification (AMB-BC) preferred.
• CPR certification required.
• Experience with EHR and database applications.
• Ambulatory office experience preferred.

Responsibilities

• Perform comprehensive RN-level clinical triage via calls, messages, and referrals using protocols and independent judgment.
• Manage provider inboxes and clinical result queues; conduct patient outreach and education.
• Oversee medication reviews, refills, and patient safety with appropriate follow-up.
• Coordinate labs, imaging, and referrals; ensure clear follow-up and documentation.
• Assist with creating and implementing clinical pathways, policies, and services.
• Engage with patients and families to support access, education, and continuity of care.
