Job summary

Augusta Health in Fishersville, VA seeks an RN Care Coordinator to provide advanced care coordination in ambulatory settings, using nursing judgment for triage, clinical decision-making, and patient guidance through the health system.

The role serves as a central clinical resource, overseeing triage, inbox management, medication oversight, and helping implement clinical pathways and services, with autonomy within established protocols.

Qualifications

• Bachelor's degree in Nursing required.
• Current and valid RN license in VA or Compact state.
• Ambulatory Care Nursing Certification preferred (AMB-BC).
• CPR required.
• Experience with EHR and database applications required.
• Ambulatory office experience preferred.

Responsibilities

• Provide RN-level clinical triage for incoming calls, walk-ins, messages, and referrals.
• Assess symptoms, vitals, acuity and determine disposition and follow-up.
• Coordinate same-day or expedited care and prioritize high-risk patients.
• Educate patients and ensure understanding using teach-back and written materials.
• Coordinate post-ED/hospitalization care, med reconciliation and education.
• Support transitions and ensure continuity of care within protocols.
