Job summary

Mayo Clinic in Phoenix seeks an RN Case Manager to work within an interdisciplinary team to coordinate the patient care plan across the continuum, ensuring utilization management and timely documentation.

The role involves leading education on case management concepts, participating in committees, and coordinating with providers, patients, families, and outside agencies to deliver high-quality, cost-effective care.

Qualifications

• Arizona-licensed RN with current license.
• One year RN Case Management experience or completed MCSHS RN Case Manager Fellowship within six months.
• 3 years acute nursing preferred; 1 year Case Management experience preferred.
• Certification CCM or ACM-RN preferred.
• Basic Life Support (BLS) competency as required by the work area.
• Other state licensure as needed by the work area.

Responsibilities

• Coordinate and facilitate the patient care plan across the continuum of care.
• Oversee utilization management, care coordination, resource utilization, and clinical documentation.
• Provide leadership through education on case management concepts, committee work, and community involvement.
• Bridge communication between providers, patients and families, and outside agencies to ensure high-quality, cost-effective care.
