Job summary

Benefis Health System is seeking a Nurse Case Manager to coordinate and implement case management across the hospital stay. You will serve as the lead point of contact for patients and families, working with a multidisciplinary team to ensure appropriate referrals and transitions of care.

Ideal candidates are RNs with current licensure, BLS, and strong clinical nursing knowledge, capable of navigating changing situations and deadlines while maintaining patient-centered care.

Qualifications

• Graduate of an accredited school of nursing (BSN preferred).
• Current state registered nurse license required.
• Current BLS certification.
• Thorough knowledge of clinical nursing skills.

Responsibilities

• Coordinate and implement case management activities across hospital stay.
• Act as point of contact for patient/family until transition.
• Collaborate with multidisciplinary team to align care with criteria.
• Attend daily care rounds and communicate changes to the care team.
• Monitor and adjust care plans and discharge resources.
• Ensure compliance with licensing requirements and organizational policies.
