Medical Center Enterprise is seeking a Care Manager RN to coordinate discharge planning, transitions of care, and case management activities to achieve optimal patient outcomes. You will collaborate with interdisciplinary teams, review records for appropriateness, and ensure compliance with standards.

The role requires Case Management experience and an ADN; preferred BSN, with 2–4 years of clinical nursing and care management in hospital or home health settings.

Formación

• Case management experience required.
• ADN required; BSN preferred.
• 2–4 years of clinical nursing experience in hospital, home health, or nursing home setting.
• 2–4 years of care management experience preferred.

Responsabilidades

• Review daily medical records to assess admission appropriateness and utilization.
• Collaborate with IDT to coordinate patient care and resolve care transition issues.
• Develop and implement discharge plans and coordinate post-hospital placement.
• Refer cases to physicians/managers when criteria not met; ensure timely interventions.
• Serve as liaison with community agencies to facilitate transitions.
• Facilitate interdisciplinary meetings addressing patient care needs.
• Maintain accurate documentation of case management activities and referrals.
• Assist patients, families, and physicians with discharge planning and post-hospital care options.
