Qualifications

Responsibilities

• Conduct comprehensive assessments and develop timely care plans.
• Collaborate with patients, families and multidisciplinary team for goals.
• Coordinate discharge planning and ensure regulatory alignment.
• Facilitate smooth transfers between units and care settings.
• Provide case management addressing access, finances, housing, and social needs.
• Document UM activities, payer updates and CMS notices on time.
