Job summary

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist in State of Illinois required.
• BLS Certification from American Heart Association upon hire required.

Responsibilities

• Selects and administers appropriate physical therapy evaluations.
• Inpatient rehabilitation.
• Develops and implements collaborative treatment plans.
• Provides patient, family and caregiver education.
• Participates with other team members in patient discharge planning through verbal and written communications.
• Provides clear, concise patient care documentation per departmental standards.
• Supervises paraprofessionals and support personnel with treatment plan administration.
