Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist in State of Illinois required.
• BLS certification from American Heart Association upon hire preferred.

Responsibilities

• Select and administer physical therapy evaluations.
• Inpatient rehabilitation.
• Develop and implement collaborative treatment plans.
• Provide patient, family and caregiver education.
• Participate in discharge planning with the care team.
• Produce clear patient care documentation per standards.
• Supervise appropriate paraprofessionals and support staff.
• Contribute to quality management and competency assessments.
• Assist in discharge and home care planning to ensure continuity of care.
