Job summary

The Charlotte-Mecklenburg Hospital Authority seeks a licensed Physical Therapist to evaluate and direct inpatient PT programs. You will plan and conduct therapy to restore function, prevent disability, and help patients achieve maximum independence.

Responsibilities include thorough chart reviews, data gathering, and documentation of progress, outcomes, and discharge plans. You will supervise PTA/ Rehab Tech and collaborate with physicians and staff to coordinate care.

Qualifications

• Evaluates patients and plans comprehensive PT care.
• Communicates progress to physicians and staff.
• Documents evaluations, tests, progress notes, and discharge information.
• Supervises PTA and Rehab Tech/Aide as required for licensure.
• Bereaves effective patient education and therapy groups when needed.

Responsibilities

• Evaluates and directs PT programs in inpatient acute settings.
• Plans and conducts PT programs to restore function and independence.
• Gathers data from medical records and patient to inform care plans.
• Documents all patient interactions and treatment outcomes.
• Coordinates treatment schedules and discharge planning.
