Job summary

HonorHealth is seeking a Physical Therapist to evaluate and treat individuals with physical disabilities, developing and updating care plans to restore function. You will work within Therapy Services at Osborn Rd, Scottsdale, with a strong emphasis on evidence-based practice and patient education.

Responsibilities include implementing individualized plans, documenting care, and collaborating with the interdisciplinary team to ensure optimal discharge planning and patient outcomes.

Qualifications

• Bachelor's in Physical Therapy from an APTA-approved university.
• Master's or Doctorate in Physical Therapy from an APTA-approved university.
• BLS certification; AZ PT license in good standing; APTA certificate preferred.

Responsibilities

• Implements an individualized treatment plan using evidence-based therapy techniques.
• Documents evaluations, progress notes, discharge summaries, and charges in the medical record.
• Collaborates with the inter-disciplinary team to clarify treatment plan and discharge needs.
• Supervises therapy assistants and delegates appropriate tasks.
