Job summary

HonorHealth is seeking a Physical Therapist to evaluate, administer, and oversee evidence‑based physical therapy services for patients with physical disabilities and functional limitations at the Scottsdale location.

You will develop individualized care plans, document progress, and collaborate with physicians, care teams, and families to restore function and improve daily living skills. Requirements include PT degree, AZ license, and BLS; a sign‑on bonus may be available.

Qualifications

• Bachelor of Physical Therapy from an accredited program.
• Master's or Doctorate in Physical Therapy from an approved program.

Responsibilities

• Evaluates and treats patients with physical disabilities to restore function.
• Develops and updates individualized plans of care with patient/family.
• Maintains accurate documentation: evaluations, treatment notes, progress and discharge summaries.
• Collaborates with physicians and interdisciplinary team to optimize discharge planning.
